Desenden nekrotizan mediastinit (DNM) nadir görülen, fakat yüksek oranda ölümcül seyreden bir durumdur. DNM, mediastendeki hayati organlarýn etrafýnda nadir görülen enfeksiyöz bir hastalýktýr. Zamanýnda taný konulmaz ve uygun olarak tedavi edilmezse sonuç fatal olur. Tedavideki gecikme enfeksiyonun mediastene yayýlmasýna yol açar. Erken taný, uygun antibiyoterapi ve cerrahi giriþimle þifa olasýdýr. Aðýz tabanýnda kötü kokulu bir sývýnýn geldiðini ifade eden 37 yaþýndaki erkek hasta 5 gün sonra ve kemikli et yemesi sonrasý yutamama þikâyetiyle acile baþvuran 57 yaþýndaki erkek hasta 7 gün sonra yatýrýldý. Her iki olgu gerekli laboratuvar ve radyolojik tetkikler sonrasý acilden ameliyata alýndý. Olgularýn tanýlarýndaki gecikmelerden sonra müdahale edildi. Müdahale sonrasý takiplerde olgular þifa ile taburcu edildiler.

DNM is infectious disease occurring in the tissues and structures of the mediastinum that spreads from the oral cavity and the neck. Unless it is diagnosed in a timely manner and properly treated, the outcome is fatal. Delay in treatment allows the infection to spread down to the mediastinum. Early diagnosis, appropriate antibiotherapy, and surgical intervention are necessary for a cure. Presently described are 2 cases: a 37-year-old man presenting with a fetid fluid discharge in his mouth ongoing for 5 days, and a 57-year-old man complaining of not being able to swallow after eating bony meat 7 days earlier. Both cases underwent surgery after the necessary laboratory and radiological procedures were performed in the emergency department. Though there was a delay in admission, both patients were discharged after an uneventful surgical intervention.
