Tension Pneumothorax in a Patient with Behçet’s Dis-ease: A Rare Condition Managed with Surgery

Behçet’s disease is a chronic multisystemic condition characterized by systemic vasculitis in which pulmonary involvement is uncom-mon, and pneumothorax is a particularly rare complication. We describe here the case of a 53-year-old male with a known history of Behçet’s disease who presented with acute chest pain and dyspnea. Chest X-ray re-vealed a left-sided tension pneumothorax. Thoracic CT revealed large apical bullae, and the patient subsequently underwent video-assisted thoracoscopic surgery (VATS) with wedge resection and partial pleural decortication. Histopathological evaluation was consistent with bullous emphysema and no evidence of vasculitis was observed. This case illustrates that tension pneumothorax can, in rare cases, develop in patients with Behçet’s disease, even in the absence of pulmonary vasculitis. VATS may be considered as a safe and effective treatment option in appropri-ately selected cases.

Behçet Hastalýðý Olan Bir Hastada Tansiyon Pnömotoraks: Cerrahi Giriþimle Yönetilen Nadir Bir Durum

Behçet hastalýðý, sistemik vaskülit ile karakterize ve birçok organ sistemini etkileyebilen kronik bir hastalýktýr. Pulmoner tutulum nadir görülür; pnömotoraks ise son derece ender bir komplikasyondur. Burada, ani baþlayan göðüs aðrýsý ve nefes darlýðý ile baþvuran, akciðer grafisinde tansiyon pnömotoraks saptanan 53 yaþýndaki Behçet hastalýðý tanýlý bir erkek ol-guyu sunmaktayýz. Hastaya, acil tüp torakostomi uygulanmýþ, toraks BT’de dev büller saptanmasý üzerine video yardýmlý torakos-kopik cerrahi (VATS) ile wedge rezeksiyon ve parsiyel dekortikasyon yapýlmýþtýr. Patoloji sonucu büllöz amfizem ile uyumlu idi ve vaskülit bulgusu saptanmadý. Olgumuz, Behçet has-talarýnda nadir görülen ancak hayatý tehdit edebilen tansiyon pnömotoraksýn, parankimal vaskülit olmadan da geliþebileceðini göstermesi açýsýndan özgündür. VATS, uygun seçilmiþ olgularda güvenli ve etkili bir tedavi seçeneði olabilir.
